We collect data about patients who use our service in order to deliver effective patient care.  We hold personal information about patients and employees.

The practice stores records about your health and the care and treatment that you receive at the practice. These records allow us to provide you with the best possible care.  Records are stored on paper and computer.  Details that we store include:

• Name
• Date of Birth
• Appointment history and pending appointments
• Clinic notes about your care
• X-rays and reports 
• Financial information relating to treatments

It is important that the details we hold for you are correct and so please keep us informed of any changes.
The person information that we hold for you will help us assess your needs and decide on the most appropriate care for you.  It will also enable us to deal with complaints and concerns promptly.  In the instance of you seeing another provider other than your usual one at the practice the information will enable them to recommend the most appropriate care.
If outside referrals are required your information will be shared with other appropriate individuals upon your consent in order for you to receive the necessary care.

Disclosure of information

Whitchurch Dental Studio reserves the right to disclose selected information under the following circumstances:

• In the event that we sell the business your details will be passed to the potential buyers of the business.
• If we are under a legal obligation to share personal data in the interests of safeguarding, safety or other similar concerns.
• In order to enforce any of our terms of use if necessary.

Access to Information

In accordance with the Data protection act 1998 you have the right to access the information which we hold about you. We request that applications for access are made in writing and your rights can be exercised in accordance with the act.  Access may be subject to a fee of up to £20.00.
